先举个例子吧。 There are five people in our team, including Yaffa(人名） = There are five people in our team, Yaffa included.
这样楼主就 应该明白些了。
原句“包括校长在内共有六个人在场” 校长是“被包括在内”，被动因此用Included。

第二题
前半句已是一个独立的句子， 故后半句做补充说明用动词的ING形式。 楼主可以参照第一题中的例句并且记住这个句型。
一二题是一个意思的两种不同表述方法。

第三题
短语 make up 的意思是“组成，构成” 起被动为“ be made uo of". 根据句意”一个由六名护士和一位医生组成的医疗队已经被派遣到洪涝灾区“。 主句是” A medical team has been sent to the flood areas" . "six nurses and a doctor"修饰medical team。 而team是由six nurses and a doctor组成，并不是team组成了six nurses and a doctor，故属于被动。 答案made up of

Make sth done 的被动形式为 be made done
应为是被他们知道,所以用被动。
