“学生在校表现证明”该如何开具

我弟弟在申请香港学校，学校那边要求给到学生在校表现证明，下面有email的内容，就是proof of the student's conduct。我找弟弟的班主任，但是班主任说以前从未写过这样的东西，叫我自己写好了再给他。可我又不是老师，我也不知道the school report showing the conduct grade该长什么样子。
